This certificate prepares students for a variety of frontline positions in public health. Students gain foundational skills and knowledge in public health, case management, communication, and complete a capstone course aligned with the core competencies of the community health worker role in Vermont. Community health workers are employed by a variety of social service and healthcare organizations and play an important role in supporting improved health outcomes for individuals and populations.
Students who complete the Community Health certificate can apply those courses to CCV’s Health Science (A.S.) program or Behavioral Science (A.S.) + program.

Program Outcomes
Students who complete the Community Health certificate will be able to: - demonstrate culturally and linguistically competent professional practices;
- apply public health principles to understand the relationship between social determinants of health and health disparities;
- practice effective case management strategies;
- demonstrate the core competencies of community health workers in Vermont;
- demonstrate preparedness for educational and career development in the student’s field of study.
